Clinical Pharmacist Jobs – Caring for Hospitalised Patients

The Clinical Pharmacist Career has gained importance in the caring of patients in hospital. Especially if they collaborate with his/her physicians and interact with the patient as well.

The aim of this research study was to asses the publications in English about the effect of the Pharmacist intervention in the care of adult hospitalised patients.

36 studies were classified according to the type of clinical pharmacist job in relation to patient caring such as the ones incorporated to medical rounds, the ones taking part in the prescription/cancellation of medication, or giving their opinion about the medications used.

Clinical Pharmacist Job Description
Clinical Pharmacists are especially trained and many specialise in Oncology, Nutrition or Psychiatry. They work in collaboration with physicians and other care givers and are different from the so called “classical” pharmacists who only devote to medicines delivery.
Clinical Pharmacists inform on possible errors in the medications delivered and notify side effects and other reactions.

Clinical Pharmacist Career – Search Studies Show Good Results
According to those 36 studies, the participation of clinical pharmacists in caring hospitalised patients reduces the percentage of side effects and the amount of errors in medication as well. As long as the patients were hospitalised, the number of patients whose medication was mistaken decreased, and also decreased the period of time they were in hospital.

When the follow-up was done by the phone, the patient received fewer medications, took a lower daily dose, and stuck at their treatment in a better way. The number of re-admissions to hospital also decreased.

According to those studies, the clinical pharmacist job should be incorporated to the care of hospitalised patients, because it improves the quality, safety and efficiency of their treatment. In future, the clinical pharmacist might have an essential role in the team of caregivers for hospitalised patients. Not only the clinical pharmacist job is vital for the patient himself, but it also accounts for a lower morbidity rate, reduced medication-related costs, a reduction in the rate of mistakes in medication, and a decrease in the amount of hospitalisation days. However, the implementation of new programs in a hospital is a difficult issue, especially if it implies assigning more economical resources.
